As a courtesy, La Sorsa Physical Therapy will ask about your insurance eligibility benefits. As the guarantor, it is your responsibility to know what your insurance covers, including deductible and co-insurance payments. You will be responsible for any charges that you may incur per your insurance plans. Co-payments are due on the date of service.
